  more than 4 weeks notice – no charge
  between 2 and 4 weeks notice – 50% of course fee will be charged.

On the 25th October 2016 the Thunder River Rapids ride at Dreamworld on Australia’s Gold Coast suffered a pump failure, resulting in the tragic deaths of four riders. This incident was investigated by the police and later a coronial inquest to determine how a tragedy like this could occur. As a process safety professional, it was interesting to follow the coronial inquest findings and then think about how process safety management may have helped to prevent this incident. Assuming that a process safety event can be defined as a high consequence or potential consequence low likelihood loss of control event, this incident had all the hallmarks, and there is a lot of common lessons for us all.

Trish Kerin, Director, IChemE Safety Centre:

 After graduating with honours in mechanical engineering, Trish spent several years working in project management, operational and safety roles for the oil, gas and chemical industries.

 Trish has represented industry on many government committees related to process safety, and currently sits on the board of the Australian National Offshore Petroleum Safety and Environmental Management Authority (NOPSEMA) and the Mary Kay O'Connor Process Safety Center steering committee.

 Trish is a Chartered Engineer, a registered Professional Process Safety Engineer and Fellow of IChemE. Trish holds a diploma of OHS and is a graduate of the Australian Institute of Company Directors (GAICD) and a Fellow of Engineers Australia (FIEAust).

 In 2015 Trish was awarded the John A. Brodie Medal by Engineers Australia for her paper on Process Safety Competency and in 2018 she received the Trevor Kletz Merit Award from the Mary Kay O’Connor Process Safety Center for her work in improving process safety internationally.
